For the first time in decades, all Health & Human Services divisions are housed at one location, the new City Hall Annex at One Jackson Square to create greater ease for residents in need of support.

Here you can find HHS licensing applications for beauty salons, barbershops, body art, kennels, pet shops, shelters and retail food establishments.

Use this form to report retail establishments and residential complaints.
