BIRTH
BIRTH CERTIFICATES
The Birth Certificates of individuals born in Jersey City after 1949 can be obtained through the Office of the City Clerk – Division of Vital Records. Requests can be made in-person, by appointment only, by mail or online through VitalChek©.

You can also obtain your Birth Certificate at the NJ Department of Health’s walk-in center located at 140 East Front Street in Trenton, New Jersey. (No Appointment Necessary) Visit their website at www.nj.gov/health/vital or dial 609-292-4087

Birth records of individuals born between 1926 and 1949 must be obtained through the NJ Department of Health.

IN PERSON

Please bring the following with you:

1. A valid (not expired) government-issued photo ID (Driver’s License, State ID)*; and

2. Cash - $20 for the first copy, $10 for each additional copy;

3. If your last name has changed since birth because of marriage/civil union or legal name change, you must bring a copy of your Marriage/Civil Union Certificate or Court Order to link the name on your current ID to the name on your Birth Certificate.

If you do not have a valid government-issued photo ID, you may present two (2) alternate forms of ID. Examples of alternate ID include:

· Vehicle registration

· Vehicle insurance card

· Voter registration

· US/Foreign Passport

· Immigrant Visa

· Permanent Resident Card (Green card)

· Federal/State ID

· County ID

· School ID

· Bank Statement (within previous 90 days)

· Utility bill(within the previous 90 days)

· W-2 for current/previous tax year

PLEASE NOTE: You must present actual copies of the above listed documents; pictures are not acceptable.

BY MAIL
You may also submit your application through the US Mail. Please include the following:

1. The completed REG 37A or 37B Form (B in Spanish);

REG 37A English
REG 37B Spanish; and

2. A photocopy of a valid (not expired) government-issued photo ID with current address (Driver’s License, State ID)*; and;

3. A Check or Money Order made payable to “The City of Jersey City” for the desired number of copies. It is $20 for the first copy and $10 for each additional copy;

4. A Self-Addressed Stamped Envelope
PLEASE NOTE: the address must match the address on your ID. If it is not the same, provide proof of your current address by providing a recent (within the last 30 days) utility bill or bank statement; and

5. If you assumed your partner’s last name because of marriage or civil union, you must include a photocopy of your Marriage/Civil Union Certificate to link the name on your current ID to the name on your Birth Certificate.

PLEASE NOTE: Do not mail your actual government-issued photo ID, alternative form of ID or Marriage/Civil Union Certificate. Only include photocopies of these documents.

ONLINE
Individuals can order their Birth Certificate online through the State’s approved vendor, VitalChek©. You can use their 24/7 online portal HERE or by calling VitalChek© directly at 877-622-7549. Online orders will be shipped via the method selected. Online orders CANNOT be picked up at City Hall. Online processing fees may apply.

MARRIAGE

You need a Marriage License to get married. A Marriage Certificate is proof that you are married.

You must apply for a Marriage License in person. To set up an appointment, please call 201-547-5150, option 3. Be prepared to provide the date and location of your marriage ceremony. During your appointment, you will complete the REG-77 Form, which is the State’s application for a Marriage License. A copy of the REG 77 Form will be provided to you at your appointment, but you and your partner should review the REG 77 before your appointment.

If you were married in Jersey City, you can apply for a Marriage Certificate either in person or through the mail.

If applying in person, you must make an appointment by calling 201-547-5150, option 3.

If applying for a Marriage Certificate through the mail, please complete the REG-37 Form and mail it to the Office of the City Clerk along with the following:

• A photocopy of a government-issued photo ID with your current address.

• A Self-addressed stamped envelope. Please note that the address must match the address on your ID. If your current address is not updated on your ID, you must provide proof of your current address by providing a utility bill or bank account statement.

• Payment: $20 for the first copy; $10 for each additional copy. Money Orders or checks must be made out to: City of Jersey City.



DEATH

If you are looking for a Death Certificate please be advised of the following:

  1. Our Office only has access to death records for those who passed away in Jersey City after the year 2000.
  2. You must prove relationship to the deceased for a certified copy of the Death Certificate. Please call our office for a list of how to establish proof.
  3. The fee for a Death Certificate is $15 for one copy, $5 for each additional.

You can make an appointment by calling 201-547-5150 or order a certificate through the mail by filling out a REG 37A form and include:

  1. Government issued ID and proof of relationship
  2. Check or money order made out to the City Of Jersey City, Treasurer
  3. A self-addressed stamped envelope
  4. Send all of the above to:
